In order to publish and present your paper at ISORC International 
Symposium and Workshop, you have to register your paper using this 
registration form. Registration should therefore be completed by
all presenters, even by students who are going to present a paper.
A person giving multiple presentations is allowed to present at most
two papers. In the latter case this person is required to pay full
registration for one paper and a reduced registration fee for the
second paper. 

Please send the completed registration form by Fax and e-mail (remove
credit card details when sending the registration data by e-mail) to the
ISORC-2004 secretariat of the local General Conference Co-Chair. The
registration should be received no later than March 10, 2004. You are
also kindly requested to send a copy of the registration form and the
bank remittance for payment by conventional postage for confirmation.
